1. By submitting this Form, Rider are agreed to responsible for their own safety.
  2. In the event of any unforseen circumstances, any cost of damage caused will be borned by Rider or their Guardian during the journey.
  3. The Organizer and their associates will not be liabled for any loss or damage caused by this test drive.
  4. Each submission is entiltled for INDIVIDUAL RIDER only.
  5. Each Rider has to submit their individual form separately.
  6. Rider agreed to receive latest news and updates from AZ E-bike or their associates.
  7. AZ E-Bike reserved the rights to amend any Terms & Conditions deemded as required from time to time.
